Patients with a history of bilateral below-knee amputation can have atypical anatomy, increasing the difficulty of the reconstruction. The technique described herein was developed to minimize the risk of graft kinking and other problems. Lower extremity bypass grafts for limb salvage in patients with a history of bilateral below-knee amputation were reviewed to identify factors that affected the results. A total of 54 lower extremity bypass grafts were performed in 41 patients with a history of bilateral below-knee amputation. Risk factors were the following: age, 68+/-8 years; extent of disease, 47% bilateral, 33% monolateral, 21% unilateral; presence of diabetes, 88%; location of the femoral artery occlusion, 49% superficial femoral, 21% common femoral, 12% profunda, 13% popliteal; and presence of a vein graft, 82%.
